CASE OF THE WEEK – “Case of removal of big Intramural fibroid (around 26-28 weeks fetus sized, weighing 1.1 kg) in 35 years old Palestinian woman” by Dr. Mona Azmy, Specialist Obstetrician and Gynaecologist, NMC Royal Hospital Sharjah

35 years old woman, presented with complaints of heavy irregular cycles with severe abdominal pain since 3-4 months. Due to continuous bleeding, she become anaemic and was not responding to medical management.

The patient is a known case of hypothyroidism on medication. She was evaluated with Ultrasound Pelvis followed by MRI pelvis. The investigations revealed large sized intra-mural fibroid (measures approximately 15×13 cm) seen extending in abdominal cavity above the umbilicus level.

The patient underwent laparotomy myomectomy under GA. The procedure revealed one large fundo-anterior intramural fibroid seen 15×13cm, (around 26-28 weeks foetus sized weighing 1.100 kg).

The anaesthesia team under Dr. Archana Nair induced anaesthesia and the surgery was performed under lead surgeon Dr. Mona Azmy.

 

The patient underwent laparotomy myomectomy, the team removed a large intramural fibroid of 15×13cm with abdominal approach in a period of 1 hour 10 minutes.

The patient was observed in post operative care and had no bleeding after which she was shifted to the ward. The patient was recovering well and was discharged on the 4th day of surgery in stable condition.
