CASE OF THE WEEK – “A case of complex elbow fracture dislocation- BADO TYPE III MONTEGGIA FRACTURE DISLOCATION RIGHT ELBOW ” Dr. Pawan Nath Specialist Orthopaedic Surgeon, NMC Royal Hospital Sharjah

A 42 years old Indian male, presented to Orthopaedic clinic with complaints of pain and deformity right elbow after fall from height.

Patient had no distal neuro-vascular involvement, no other co-morbidity and his functional and dominant hand was right side.

Pain medication was given and patient was immediately put in above elbow posterior splint right side, xray advised showed complex elbow fracture with posterio-lateral dislocation with fracture of radial head and proximal ulna shaft communited fracture. Further CT scan was advised which showed more than 50% of radial head crushed and above finding.

Patient underwent planed surgery next day with Open reduction and fixation of ulnar shaft fracture with plate and screw and replacement of radial head with radial head prosthesis.

The challenge in this case was the prevention of stiffness post-op and have a good functional movement of right elbow.

With our efficient physiotherapy team, positive involvement of patient and early mobilization post surgery at 2 week the below result were able to achieve.

After 3 month post-op patient is having full range of movement right elbow, no pain and had rejoin his regular duties.
